Assuming no history of bipolar/manic depression/suicide attempts, I would think United Healthcare and Humana would both be options here. Not sure about the rest of the companies in FL.
The only concern is the anxiety for the insurance company. No companies in FL cover pre-existing mental health, so he will be out of pocket for those costs. Go with a company that can rider the condition like UHC, or Assurant Core Med plan. Aetna and Cigna will be more difficult b/c they do not have ability to rider conditions, and you will get an un-necessary rate increase. I don't see any chance of a decline if you apply for the above 2 companies.
