It would be easy to put a two question quote box in a widget. You can use that "quote starter" to redirect a visitor to a quote form.
You can't do a full fledged quote form on a WP post. It will take up too much room.
You are going to need someone who knows HTML and a little programming to create your quote form no matter what you do unless you redirect to something like Norvax.
I have had success with a floating quote box on my disability insurance blog. It seems to work better than a static form.
