Any Simplified Issue That Will Take 55 Y.o. Male W/ Quad. Bypass and Still Smoking?

We have a 55 year old male smoker, 6'0" 210 lbs, had a heart attack and quadruple bypass at age 47, full recovery. Takes blood pressure and cholesterol meds, takes 2 diuretics for water retention (may be related to the heart condition). Is there any simplified-issue term or permanent carrier that will accept him? We have checked with ~10 fully underwritten carriers and they are all Table 6 and up smoker rates. Just trying to compare what else may be out there.
Edit - We would also prefer to use A+ or A carriers if there are any.....A- would be a last resort.
