I spoke to a lady today that is switching to a med supp because her Coventry PFFS is toast as of Dec. 31. I needed the dis-enrollment letter for her to be taken without health questions asked. She thought she threw it in the trash so I called Coventry to see if they could mail her another one and they said that they had only mailed it out yesterday so there is no way she could have it yet. Well guess what, she found it in a stack of bills. It is a 5 page letter that was mailed out to her(It says on the fromt page for her to keep it as proof of the dis-enrollment). Is this the actual form or is there another one?
