Rules on Issuing Cert of Creditable Coverage on Request?

My new client was aware that there would be charges for lack of creditable RX coverage due to a gap in insurance for the now Medicare covered spouse.

Client is possibly terminally ill, but soldiering on. Yesterday, I was unable to produce a perfectly good contact # for UHC group benefits, the carrier that was his small group insurance coverage ending about 6 years ago. He has lost the number.

Needs the Certificate of Creditable coverage asap to avoid permanent upcharge on Medicare Drug plan for the 3 years of that coverage. There are still about 2-3 years of no creditable coverage, can't be helped. Covering 3 years will be a big benefit, however.

He says he requested one about a month ago, they said they would mail it, but has not been received. We did find a number online and he was going to call them.

I wonder if there may be a requirement to produce those in a timely manner.
