I am currently covered under my husband's employer's health plan. Beginning in 2017 I can be no longer covered, if my employer offers coverage.

What kind of proof will my husband's employer ask for?

Typically, they will ask for a spousal affidavit to attest to the fact that you don't have access to health insurance where you work. On it, they could ask for your employer's name, address and phone number.

They will ask for a letter from your employer, or provide a copy of the offer letter (which would contain that you do not receive benefits). You will sign an affidavit that your insurance doesn't provide insurance.
