Cardiomyopathy, Pacemaker, Diabetes

I have a 73 yr old with the conditions listed above. Diabetes is treated with medication only
Cardiomyopathy was treated with an ablation over 2 yrs ago. Pacemaker implanted 3 yrs ago.
Still takes Carvediol and warfarin tho.

Any ideas on who will take this. I am at Trans STD. Right now
