Standard Life and Accident out of Dallas, TX has good market for you, issue ages 17-69, with a small list of excluded occupations.

AGLA (American General Life and Accident) is based in Nashville. You can get a brokers license with them. Google for a local office.
AGLA has a decent plan but the problem with AGLA is the renewal commissions are the worst I have ever seen for a supplemental health product. 1 1/2% with a 1 1/2% service fee and the service fee is not vested. Most compnies pay in the 12% - 15% range with full vesting.
What about United American? They always had a standalone cash cancer plan.
