I think the OP meant what is causing uncontrollable high healthcare costs. You pay for the right to make Dr copays and have a low deductible. It is usually a choice, the exception being group plan choices.

So what? These things aren't really insurance anyway, just add-ons for predicted, non risk events. People lump it in as insurance and as something they are entitled to (whether they realize their premiums are higher because of it, who knows?). It is still one of the drivers of higher costs.
