What is the best method to list out potential new clients and document the calls you make and if you need to follow up? I would like to have something that reminds you to follow up with that particular client on that day. what does everyone use?

Any database or contact management software will do that. If you are a one man army ACT works very well. Also YIO (see Greensky on the forum) is a great simple one.
 
There is a very good guy on here that hipped me to SalesEnvy.com @Hopwood04 is also an experienced producer and will take you under his very large wing. While I haven't used SE much it has bigtime coldcalling/dialing/voicemail juice.
