I would recommend seeking out an underwriting prescreener through an IMO or BGA. I just had a friend who was looking for a $500,000 face value, 20 year policy for a guy who had a quadruple bypass heart surgery. I recommend he look in to Simplified Issue policies, but also referred him to a BGA that I work with who has an underwriting pre-screening service. They take the info, contact their carriers, and find the best possible policies for someone in that condition.

WOW! The trend that will disturb underwriters most is that the nature of the vast majority of his offenses involve behaviors that consistently lead to premature deaths. A trial app may yield better results than the gloom and doom I am relaying, but IMO, ANY coverage in this situation would have to be considered victorious.
