If you get licensed in your state, does that mean that you can only sell insurance to people that live in your state, or that you can only operate out of your state ?

Only in your state. You must get a non resident license for each additional state you wish to sell in.
You have to be licensed in your home state with a Resident License and in any other state you sell in with a Non Resident License.. Once you have your Resident License you can apply online for most other states. Most if not all accept your credentials and there is no further testing required. I use Sircon Corporation | Licensing & Compliance Automation for the Insurance Industry to get my Non Resident Licenses. YMMV in some states, that is how it works in mine. I have licenses in 6 different states right now.

How much does it cost to get appointed in another state and is it pretty easy to do?
It is pretty easy to get non-resident license. The cost vary from state to state. Some state does not require to pay the fee to be get licensed but many state charge money from 5 dollars to 70 dollars.
Once you receive your state license, it is usually easy to get a non-resident license in any other state you want to sell into, it just costs a few bucks. You can usually do it online. Some states require that you submit your finger prints. Each state license department typically provides extensive instructions on their website.
