I was doing a little research on the Humana 's premiere plan, Portrait Share 80 and in their brochure, under the covered benefits included with Physician Services there's a There are two bookmarks labeled (12) and (15). (12) says "This benefit does not cover MRI, CAT, EEG, EKG, ECD, cardiac catheterization or pulmonary function studies."

What the hell does that mean? Does the plan say that they won't pay for these services, or they won't pay for these services if a primary care physician orders it?

I'm confused...

The benefit for physicians services says the first $200 of lab and x-ray is covered at 100% then 80% after deductible.

The (12) means that specifically MRI, CAT, EEG, EKG, ECG, cardiac catheterization or pulmonary function studies is not covered/included under that $200 benefit.
