Maybe a picture of a Dr. in his lab coat for Part B?

I have seen one where it lists A, B, C and D on the left but leaves C blank and then on the right lists A, B, C and D and explains what Part C is and leaves A, B and D blank. That way they basically know there are 2 distinct ways to get their coverage. And beside Part B it listed $104.90 so they are aware of this premium in addition to whatever plan they choose.
 
I'm sure you know about these already, but why not just download the Medicare and You book from Medicare.gov and email it to him? Or if you know his address, have one mailed to him? They're free.

Pages 15 & 16 give a pretty good overview of Medicare and how it works with MAPDs. If you're a Med Supp guy there's Choosing a Medigap Policy book.
 
Do pieces like this have to have carrier or CMS approval?

Generic marketing pieces that fit the CMS definition for such do not require advance approval.

I would just use page 16 from Medicare and You. Bob's handout has good info, too.
