When Moving Out of a Service Area, Who Sends the SEP Letter?

I have someone that their moved permanent residence from Florida to NJ. Under-65 Medicare beneficiary. They were enrolled in an MAPD first year of eligibility. Once he moved, he was enrolled in a PDP-only in NJ under the SEP and approved effective Sept 1, but now trying to get a Med Supp also and underwriters are requiring a letter stating specifically he has eligible SEP moving out-of-service area.

My question is, who is responsible for sending the letter? Is the Carrier expected to send letter, or CMS? I have confirmed with CMS that he is enrolled in the PDP eff. 9/1, which automatically kicks him out of MAPD, but they are pointing the finger that the actual SEP letter is generated from the Carrier. The Carrier says the SEP letter generates from CMS. Meanwhile, 63 days is dangerously close to passing.

Any advice greatly appreciated.
