Lifeshield is very inexpensive and offers the best copay arrangement Ive seen for any doctor in network including specialists, its on the largest PPO network Multi-plan. They offer very customize-able deductible and coinsurance packages. (Expert tip: you can cancel a few of the ancillary services like teledoc and RX helpline to open up budget for more important allocation of monthly premium. I often add an accident or supplemental coverage to cover OOP.)
You have to be careful with the Multiplan PHCS there are many PHCS/ multiplan networks and just because the provider takes it don't be so sure it takes the one your a part of. You need to look up the provider
