Thought I'd share something I've used successfully for years.

The State of New York places a surcharge on inpatient non-Medicare serves rendered. The surcharge was created by legislation named the New York Health Care Reform Act (1997). and is managed by the NY State Department of Health.

Fully insured groups are charged via the retention/expenses portion of their premium. Self-Funded groups get charged directly and individually.

What makes this somewhat interesting::
1. Each S-F employer must complete an "Election Form"
2. A summary of these Election Forms is public information.
3. The line item summary includes Election Date (usually, but not always, renewal date), Phone Number, Contact name, etc.
