Life (other than Variable Life), Accident and Health, Long Term Care, Medicare Supplement, Disability Income, Fully Insured Group A&h $500 Deductible applies to these products



For Level 2 , you add $75 for 12 months.

Level 2 is everything in level 1 plus Fixed Annuities, Equity Indexed Annuties. A $2,500 deductible applies to this products..


To finish off the rest of the year is $350.55 level 1
