Creating Forms

Anybody know of a good site for creating forms? Preferably one that specializes in the Insurance industry, if one exists. I snooped around in here and didn't come across any threads.
 
What type of forms are you referring to? Lead generation forms? Policy updates? Something else?

For starters, I'm looking to create a new Fact Finder for myself. I've been using one provided by an IMO, and I'd like to create my own. Preferably one that is fillable online.

Might be late to the thread, but have you tried the Google forms?
I believe now it has customization possibilities. With GForm you can create a form of your choice and design, rather than going for a conventional format. Also, you can try Canva, but it has limitations I guess. Do try these, please if you are looking for online/printed forms.
