The voice-signed Gerber app (that Matt mentions) doesn't require an email and you just read disclosures and submit the same way you would an e-app, the portal just greys out the e-signature areas.
The catch is you have to be 100% compliant with Gerber's guidelines by letting the person know the call is being recorded (at the beginning of the call) and read disclosures verbatim. It's made for call centers used to monitoring everything, not independent agents writing a few apps/month.
